What data do we collect?

Wokingham Acupuncture Ltd collects the following data:

  • Personal identification information (Name, address, email address, phone number)

  • GP name and address

  • Date of birth

  • The presenting complaint and symptoms reported by you

  • Relevant medical and family history

  • Wokingham Acupuncture’s clinical findings

  • Any treatment given and details of progress of the case, including reviews of treatment

  • planning

  • Any information and advice given

  • Any decisions made in conjunction with you

  • Records of your consent to treatment, or the consent of next-of-kin

  • A permanent attendance register which records patients attending the clinic for RIDDOR

We collect this data via our registration and full traditional diagnosis form.

How do we use your data?

Wokingham Acupuncture Ltd collects your data so that we can

  • Plan and carry out relevant treatments

  • Contact you with appointment reminders

  • Email you lifestyle information on relevant exercise plans or dietary suggestions

How do we store your data?

Wokingham Acupuncture Ltd securely stores your data in a locked filing cabinet on the premises and
on a password protected computer.

Wokingham Acupuncture Ltd will keep your data for 7 years after treatment ends in compliance with the BAcC regulations. For a child, records will be kept for a further 7 years after their 18th birthday (aged 25 years). Once this time period has expired we will delete your data by shredding the paper file and permanently deleting the data from the computer.

For Covid-19 track-and-trace data we will keep the list including your name and contact telephone number for 21 days from your visit to the clinic.

What are your data protection rights?

  • The right to access – you have the right to request copies of your personal data. We will charge you a fee of £25 for this service

  • The right to rectification – you have the right to request that Wokingham Acupuncture Ltd correct any information you believe is inaccurate or incomplete.

  • The right to erasure – you have the right to request, in certain circumstances, that Wokingham Acupuncture Ltd erase your personal data.

  • The right to restrict processing – you have the right to request, in certain circumstances, that Wokingham Acupuncture Ltd restrict the processing of your personal data.

  • The right to object – you have the right to object, in certain circumstances, to Wokingham Acupuncture Ltd processing your personal data.

  • The right to data portability – you have the right to request that data you have provided to Wokingham Acupuncture Ltd is transferred to another organization or directly to you.
